About HMIS
Colorado Coalition for the Homeless (CCH) is the implementing organization for the Homeless Mangement Information System (HMIS), under the direction of the Metropolitan Denver Homeless Initiative (MDHI) Continuum of Care (CoC). HMIS utilizes a computerized data collection tool designed to capture client-level information over a period of time. HMIS collects information on the characteristics and service needs of persons experiencing homelessness, or who are at risk of becoming homeless. It incorporates a set of policies and procedures around client confidentiality, data collection, computer entry, and reporting. HMIS is designed to aggregate client-level data to generate unduplicated counts of clients served in the community. By enrolling homeless service providers into HMIS, it is our goal to utilize HMIS to benefit the community through:
- Streamline referrals, intake, and assessments for consumers
- Increase coordination of case management between workers
- Assist with benefits access eligibility
- Evaluate consumer progress and outcomes
- Increase agency efficiency in internal and external coordination of services
- Accurate reporting to agency funders, boards, and stakeholders
- Increase community understanding of homelessness
- Reduce service gaps within the community
- Develop better policies to prevent and minimize homelessness in the community
- Meet federal reporting requirements
